Digestive Health is a clinical partnership between UT Health Austin and Ascension Seton. The Digestive Health clinical team provides comprehensive, digestive care for adult patients. This position is a collaborative hire between the Department of Health Social Work and the Department of Internal Medicine. The first of its kind in the nation, the Department of Health Social Work is a collaboration of Dell Medical School and the Steve Hicks School of Social Work at The University of Texas at Austin. Jointly operated by both schools, the department creates a formalized structure of collaboration to support their shared interprofessional missions.PurposeTo support patients in improving their overall functioning by providing psychotherapy and case management services using a patient-centered, collaborative approach with an interdisciplinary team. To formulate and implement comprehensive assessments and individualized treatment plans to address biopsychosocial needs for patients experiencing various medical conditions including chronic illness, adjustment disorder, depression, anxiety, alcohol and substance abuse disorder, and obesity.ResponsibilitiesLCSW completes a bio-psychosocial assessment of patients. The applied biopsychosocial model and approach will include cognitive behavioral therapy, motivational interviewing, supportive therapy and/or other techniques and modalities as clinically indicated. Completes relative evaluations and treatment plans, and provides psychotherapy, through a series of face-to-face sessions. Maintain written documentation and generate charge capture for all therapy services rendered, including the initial assessment, follow-up visits, and discharge summary, consistent with organizational standards.Assists patients/caregivers by (a) sharing information and perspectives on how other life factors are affecting chronic illness or other medical condition and (b) helping patients/families to build coping strategies, psychological skills, and ways to improve your daily function and quality of life. Provide case management as needed to include connecting patients to needed resources such as prescription assistance programs and other resources in the community.Participate in value-based research and research for patient care operational improvements working closely with clinic staff and faculty in partnership with the Department of Health Social Work, Dell Medical School. Participate in screening, tracking, and reporting of patient outcomes and social work interventions.Provide discipline specific knowledge to interdisciplinary team around mental health, cultural competency, social determinants of health, and innovative ways to approach chronic illness in patients. Participate in social work/departmental meetings, continuing education opportunities, academic and research opportunities to optimize patient best practice and interdepartmental collaboration.Develop and maintain working relationships with behavioral health providers in the community to create a network of aligned providers.Required QualificationsHold a Master of Social Work (MSW) degree from an institutional program certified by the Council on Social Work Education (CSWE). Be licensed as a Licensed Clinical Social Worker in the state of Texas. Have post-degree clinical experience in assessment, evaluation, treatment planning, and psychotherapy. Demonstrate the ability to communicate and collaborate effectively with a variety of other health care professionals (i.e., physicians, psychologists, social workers, nurses, physician assistants, physical therapists, dietitians, pharmacists, medical assistants). Maintain a positive, compassionate, and professional attitude when engaging with patients and their family members. Maintain a highly collaborative approach to a diverse and inclusive cohort of clinical team members, as well as the local, regional and national professional communities. Possess excellent interpersonal skills; written and oral communication skills; organizational, planning, follow-up/follow-through, and time management skills. Possess strong analytical and problem-solving skills. Ability to work in a fast-paced, multi-provider practice environment.Relevant education and experience may be substituted as appropriate.Preferred QualificationsHave a minimum of three years of post-degree clinical experience. Experience working in a healthcare setting.Have experience delivering care as a member of an interdisciplinary team. Have a working knowledge of and/or experience with tobacco cessation, Alcohol Screening and Brief Intervention, and Motivational Interviewing, and value-based healthcare delivery.
