Funding is expected to continue.Activities of the Care Navigator include coordinating care with mental health, primary care, and social services providers in partnership with other community organizations and supporting young adults with mental health challenges through skill building, advocacy, and system navigation. This position will be assigned on-campus with an expectation to report to work on-site during the scheduled work week. As a reminder, COVID-19 social distancing and safety guidelines are in place, and employees will be expected to comply with the safety protocol as outlined. PurposeThe Care Navigator will serve as a key member of a multidisciplinary team in an innovative, pilot mental health clinic that is focused on improving mental health services for young adults. This clinic is being built out of an exciting partnership between Austin Community College and the University of Texas Dell Medical School. The Care Navigator will have the opportunity to contribute to and build this first-of-its-kind clinic in Texas.ResponsibilitiesAssist in skill-building through coaching and one-on-one work with young adults. Advocate and assist with problem-solving that will help the client access and utilize mental health supports and services. Conduct community outreach. Develop relationships with diverse community members and organizations. Maintain regular communication with community partners.Attend agency staffings, team meetings, and other meetings/appointments as assigned. Complete necessary reporting requirements and ensure achievement of performance measures for funders and stakeholders. Maintain written documentation for all therapy services rendered, including the initial assessment, follow-up visits, and discharge summary, consistent with organizational standards.Required QualificationsHigh School Diploma and Five (5) years of community/health or related experience OR Bachelor's Degree in a related field. Demonstrated high-level interpersonal and communication skills. This person should have good written and verbal communication skills and strong basic computer skills, which include the ability to document services in a clear and effective manner. He or she must function well independently and as part of a team. Excellent community engagement skills, including ability to reconcile differences. Strong customer service mindset in working with internal and external partners. Experience working with young adults from diverse backgrounds (people experiencing homelessness or violence, formerly incarcerated, people with disabilities, physical, and mental illnesses). Some knowledge of community-based resources including health and social services. Ability to work independently and as a part of a team in implementing project goals. English proficiency required.
